This sounds like it could be several things.
1) Your analog functions in the controller are going bad.
This can cause a lot of goofy signals that disrupt normal controller usage.
2) You are using an extension cable that has gotten dirty or is
also going bad. These things can cause some serious disruptions.
3) Your controller port is going bad or has a loose connection.
4) Your controller buttons, control port, or plug ends need to
be cleaned.
5) Some newer games that utilize the L3 and R3 buttons have
been found to cause issues with the basic controller functions.

I would start by cleaning everything, then checking the other options.
Mine did similar stuff a little while back and it was because of
a faulty extension cord. Well, faulty after my new puppy tinkled on
it. >_<!
