The first section is for the use of those who have already pre-reserved
their email id before. This section does not apply to you.
The second section is for the use of new applicant. You should enter 3
choices of the email address and the forwarding address.

Wrong Joe. The one who wrote the mancow.com email is Joe Petrow, not Joe
Monson. They are both in Japan, but both are not living in Tokyo.
