If you're doing a summary of the TPBs, then the TPB version 
is correct. Actually, from Keiichi's perspective, the manga
and OVA are virtually identical. The major difference is 
with Belldandy. In the manga she has no previous relationship
with Keiichi, while in the OVA she does.

Yes, she disappears and is never heard from again.
