I got a few request about this, so here is the instruction:

Within each CD package is a leaflet explaining how to apply for the
extra CD.  The name of the CD will be printed on one of the 4 squares on
the bottom of the leaflet.  You need to cut out the square containing
name of the CD, stick all four of them onto a postcard (white, 10cm *
14.8cm) and write the following info on the same side:

o Your Address
o Your Name & Age
o Your telephone number

Use the other side for postage and Pony Canyon's address:

If you can't write Japanese, just photocopy/enlarge the sample on the
leaflet (the left side, with 105-8487 on top) and stick it to the post
card and send it to Japan.

The deadline is Jan 31.
