Those are pictures in the doujinshi AAH! MY GODDESS series produced by
C-ARTS (Yamaguchi Shunichi).  Contrary to most people's thinking, AAH! 
MY GODDESS is non-H.

They have : Urd in Sailor Jupiter's custom (color), Bell-chan/Moon + 
Skuld/Mercury + Urd/Mars (color, this is the pic most of you are talking
about, and someone had posted a b/w scan very long time ago), Skuld in 
Anna Millers custom (b/w), Bell-chan in Anna Millers (b/w), Bell-chan
as Moon jumping out of the mirror (b/w), Belldandy/Mars + Skuld/Moon 
(b/w).

IIRC, the last AAH! MY GODDESS was issued in 1995.  If you want to get
one, send me a private email and I will give you the address.  If you 
do not know Japanese, forget about it, Yamaguchi-san is not good in 
English.
